Before making the call, please ensure that you have the following documents and information:
- A copy of the policy and/or certificate number.
- The policy holder’s full name, date of birth, phone number and address.
- Date and cause of death, or accident.
- If you need more information on an insured’s policy, you will need to provide a will, or have your power of attorney documents ready for authorization should you need to make decisions on a living claim.
